Baystate Health, western Massachusetts’s premier healthcare provider, and home to Baystate Medical Center (BMC), a Magnet Hospital for Nursing Excellence is looking for full time Nurse Clinician for our Wound Care Team.

What you will do:

• You will work collaboratively with the staff and the providers to deliver care and education to the Wound Care patients and their families and assist with clinical decision making/problem solving and to provide hands on treatment of the patient with chronic wounds in the ambulatory setting.

• You will be responsible for program compliance, outcomes, and functional status: strategize with patients for reaching wellness goals and overcoming obstacles; following unit specific protocols/guidelines; follows WOCN guidelines related to the care and treatment of various types/stages of wounds; monitors for signs of healing/infection; develops a comprehensive assessment plan and monitors the patient's compliance related to the treatment plan.

• Nursing Practice Assessment: Assesses the health status, physical, social, and emotional needs of the patient and documents the assessment according to established protocol; collaborates with the health care team in the implementation of the care plan.

• Patient Education: Ability to perform patient teaching appropriate to the developmental stage and the diagnosis for which the patient is being treated; assesses the educational requirements of the patient and provides education on (i.e., diet, control of Hgb A1C, smoking cessation, exercise, compliance with treatment plan); communicates knowledge of diagnosis, wound type, wound management, and treatment modalities.

Learning and mentoring is at the core of our professional practice model. Baystate sponsors nurses obtaining certification and supports nurse empowerment through attendance at national conferences

SCHEDULE:

• Full Time Days 40 hours Monday- Friday

• Schedule 4 (10)’s 7a-5:30pm

• Location: Baystate Medical Center Springfield

QUALIFICATIONS:

• Successfully passed Board of Registration in Nursing Examination and licensed in the Commonwealth of Massachusetts.

• Current American Heart Association Health Care Provider CPR certification required.

• 3-5 years of experience in the following preferred: dressing indication/application; ointment application; debridement; vascular assessment (manual, doppler, ABI); compression therapy; negative pressure wound therapy; wound culture; ostomy care; patient educations; assist with procedures (bx's, application bio-engineered skin substitute)

• Education: BSN required

• WOCM wound and Ostomy certification required
